Dear Health Care Provider,
The Cardiac Neurodevelopmental Outcomes Consortium, which is a national interdisciplinary collaborative group with the goal of improving neurodevelopmental outcomes in children with congenital heart disease (CHD), is conducting a survey on neurodevelopmental screening in children with CHD. In 2012, the American Heart Association released a statement regarding the evaluation and management of developmental problems in children with CHD.
You have been selected for this survey to help us better understand what type of developmental evaluation children with congenital heart disease are receiving and how these guidelines are being implemented. There are 20 questions and it should take only a few minutes to complete.
The survey is optional and all responses are anonymous. Your participation is voluntary and will help us to identify strengths and weaknesses within the current evaluation process. The survey can be viewed by clicking on the link below and will be submitted electronically.
