Karen writes:

“As I’m sure you are aware, there is a huge problem with loss to follow-up with adults with CHD.  In my work on transition, I have learned that < 50% of 13-25 year olds prefer to receive information face to face from clinicians.  Since that is probably the most common method used to communicate transition knowledge to our patients, I am interested in knowing what people are doing, what resources they use.  I think it may identify a huge need and potentially one that the SPCN membership could address.  Nurses always have been the experts when it comes to developing educational materials!”
